In preparation for our new grant application to the U.S. Administration on Developmental Disabilities, we need your help in identifying the needs of Ohioans with developmental disabilities and their families. We are especially interested in surveying persons with developmental disabilities, their families and providers to learn about services and supports needed across the following areas: (1) Early Intervention/Early Childhood Education
(2) Special Education, Transition Services and Adult Life Services
(3) Health and Healthcare Services
(4) Behavioral and Psychiatric Care

If you are a parent, guardian or sibling, we ask that you focus your responses on a specific person with developmental disabilities with whom you are most familiar. You can find the survey at this link: http://www.surveymonkey.com/s/Nisonger_Needs
