Description of Study:
The purpose of the current study is to investigate the relationship between ASD symptoms and functional impairment and to gain a comprehensive understanding of the co-occurring factors (e.g., anxiety, behavior, etc.) that may contribute to this relationship.

This study is comprised of several questionnaires to be completed by participants. All questionnaires will be entered into Checkbox, an online survey program, through the University of South Florida’s portal supporting this software. On this program, participants will first be given the details of the study and asked to click a box indicating their consent if they express interest in participating in an investigation examining ASD and functional impairment. Then they will be prompted to follow additional links to complete study questionnaires.

Identities of participants will be kept completely anonymous. Participants will be required to read the informed consent form in its entirety and attest to the parameters specified before checking the appropriate box indicating their selection to participate in the study. Failure to check the specified box and consent to the parameters of the investigation will exclude the participant from study participation and conclude the survey.

Participants who consent will be directed to a screen that will enable them to begin to complete the questionnaires listed below. Once the participant has submitted his/her answers, the data will be stored on a password-protected account on the portal. The IP information of participants will not be stored. This will ensure that all of the data remains confidential and cannot be tracked back to the participant’s original IP address. As results come in, they will be deleted from the account on http://hsccm2.hsc.usf.edu/checkbox and stored in a database on the secure, password-protected network drives in the Rothman Center, and will only be accessible to the Rothman Center staff.
